Social Services Specialist

About the role

The Social Services Specialist will work full‑time on‑site at Polyclinic Medical Center in Doha, Qatar. The role focuses on assessing patients’ and families’ psychosocial needs, providing counseling, and developing individualized care plans to support wellbeing and access to services.

Key responsibilities

• Conduct comprehensive psychosocial assessments for patients and families.
• Develop and implement individualized care plans that address identified needs.
• Coordinate case management activities and ensure continuity of care across medical and community services.
• Connect individuals with appropriate community resources and advocate for patient rights.
• Maintain accurate documentation and participate in multidisciplinary team meetings.
• Contribute to program development, quality improvement initiatives, and community outreach efforts.

Required profile

• Demonstrated expertise in Social Work, Human Services, Psychology or a related field.
• Strong case management and social services experience with diverse patient populations.
• Experience in community outreach and building relationships with local organizations.
• Knowledge of healthcare or clinical settings and psychosocial issues affecting patients and families.
• Excellent communication, empathy, and active listening skills in a multicultural environment.
• Ability to work collaboratively in multidisciplinary teams while maintaining professional boundaries and confidentiality.
